Grounded in our commitment to Client and Family Centred Care the key focus on the RN and RPN roles are to provide and coordinate home health nursing services to diverse group of individuals, families and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.
The Adult Visiting program services clients who are 18 years old and above and referred through HCCSS. This is a visiting nurse opportunity, where driving is required to support clients with complex care and specialized nursing skills at home. In this program, nursing services such as wound care, IV therapy, injections, removal of PICC lines, and chemo disconnections, are provided. Nurses will attain specialized clinical skills (G-tube feeding, Total Parenteral Nutrition (TPN), Occasional pain management, and symptom management) to service the complex clientele. Additionally, completing mental health support and check-in with clients is also required.
Key Areas of Accountability
VHA’s Registered Nurses and Registered Practical Nurses are required to do the following tasks, but not limited to:
Assess, Monitor, and Makes Care Planning/Care coordination
Evaluates care plan interventions through on-going reassessment and evaluation results.
Ensures the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
Demonstrates professionalism, leadership, judgement, and accountability in interactions with client and families.
Supports client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Informs nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
Maintains documentation standards according to the College of Nurses of Ontario.
Required Qualifications:
Registered and in good standing with the College of Nurses of Ontario (CNO).
Membership of your Professional Association preferred (RNAO/WeRPN).
RN diploma/degree or RPN diploma from a recognized university or college.
Hold a valid First Aid or CPR Certification.
A moderate level of wound care or venipuncture knowledge and experience is an asset.
Acute Care and Medical-Surgical nursing experience is an asset.
Knowledge of community nursing is an asset.
Demonstrate effective communication (written and verbal) and documentation skills.
Demonstrate reliable attendance, punctuality, and availability for the role.
Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is required.
