About VHA Nursing Programs:
The Adult Visiting Program provides specialized care services to individuals aged 18 and above who require complex care and specialized nursing skills within a home setting. Referrals to this program are facilitated through HCCSS, and our nursing staff operates within the Durham, Toronto Central, and Central regions of the Greater Toronto Area. As part of this visiting nurse opportunity, travel and driving are essential components of the role.
The Palliative Care program delivers home nursing services dedicated to end-of-life care for individuals, families, and groups. Our nurses are committed to enhancing the quality of care for palliative clients, with a focus on effective symptom and pain management to improve their overall quality of life. Our nursing services extend to the Durham, Scarborough, Central, and Toronto Central areas. As part of this visiting nurse opportunity, travel and driving are essential components of the role.
The Child and Family program is designed to support families caring for children with medically complex needs, spanning an age range from 1 to 25 years old. These clients require specialized care and nursing skills, whether at home or within a school environment. Our services extend to providing bus escorts for school transport and accompanying clients to medical appointments (subject to HCCSS approval). This program offers both shift-based and visiting nurse opportunities to accommodate various care needs and schedules for our clients, families, and groups.

Key Areas of Accountability
VHA’s Registered Nurses and Registered Practical Nurses are required to do the following tasks, but not limited to:
Assess, Monitor, and Makes Care Planning/Care coordination
Evaluates care plan interventions through on-going reassessment and evaluation results.
Ensures the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
Demonstrates professionalism, leadership, judgement, and accountability in interactions with client and families.
Supports client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Informs nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
Maintains documentation standards according to the College of Nurses of Ontario.
Required Qualifications:
Registered and in good standing with the College of Nurses of Ontario (CNO).
Membership of your Professional Association preferred (RNAO/WeRPN).
RN diploma/degree or RPN diploma from a recognized university or college.
Hold a valid First Aid or CPR Certification.
A moderate level of wound care or venipuncture knowledge and experience is an asset.
Acute Care and Medical-Surgical nursing experience is an asset.
Knowledge of community nursing is an asset.
Demonstrate effective communication (written and verbal) and documentation skills.
Demonstrate reliable attendance, punctuality, and availability for the role.
Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is required for visiting roles.
